Trans-Anal Extrusion of a Ventriculoperitoneal Shunt in a Toddler with Posterior Fossa Ependymoma: A Rare Imaging Diagnosis

Trans-anal extrusion of a ventriculoperitoneal (VP) shunt is an exceedingly rare complication that may present with minimal symptoms. We report a 28-month-old boy who presented with catheter protrusion from the anus while remaining clinically stable. Abdominal radiography demonstrated intraluminal migration of the distal catheter with a looping configuration in the pelvis, a pathognomonic finding. Laparoscopy confirmed transverse colon perforation. This case highlights the critical role of radiography in diagnosing this rare entity to prevent life-threatening infectious complications. Early recognition and intervention are essential.
